When additional clinical documentation is needed to make a benefit determination, Delta Dental of South Carolina will indicate that on your ERA or EOB. A new claim form is required with the submission of any requested documentation. Attach x-rays, photos and any narratives to a new claim form and submit it to us for benefit determination. Clinical documentation submitted with the EOB is no longer accepted.
